Surgical Treatments

Surgical treatments include grafting of fat to address volume loss and contour deformities along with facelift and necklift surgery.

Facelift

Facelift surgery addresses excess temporal, mid/lower facial, and to some degree neck skin resulting in smoothing out wrinkles. In addition, it repositions soft tissues which have sagged resulting in resulting in a change in facial shape, loss of cheek prominence and the appearance of ‘jowls’. A further aim is to restore harmonious facial proportions.

Neck lift

Neck lift surgery is often performed in conjunction with facelift surgery to address skin excess, vertical lines (platysmal bands) and double chin. Where appropriate, liposuction techniques may be used in conjunction.

Lipofill (Fat Grafting)

Fat grafting involves transferring fat cells extracted by specific liposuction techniques from donor sites such as the abdomen or thighs to augment lip volume, address facial contour deformities and hollow cheeks.

Approximately two thirds of the injected fat survives long term, meaning multiple treatments can be performed. The procedure is performed under a general anaesthetic or with local anaesthetic and sedation in an accredited hospital as a day procedure.

The optimal outcome of these procedures is a rejuvenated, natural result aimed at turning back the clock, rather than a radical alteration of a patient’s appearance.

Non-Surgical Treatments

At NorthEast Plastic Surgery, all non-surgical treatments are performed personally by your surgeon or a highly trained and experienced cosmetic plastic surgery Nurse or Dermal Therapist to ensure bespoke treatment, achieving optimal results and ensuring continuity of non-surgical and surgical management.

Non-surgical treatments commonly include treatments to reduce the action of facial muscles that cause wrinkles, along with treatments that address deep wrinkles and contour irregularities. Other treatments include dermal therapies such as needling and peels, and cosmeceutical skin care regimes to maintain your skin in optimal condition.

These treatments can improve pigment spots, uneven skin tones and redness or stubborn wrinkles and lines.

Why Consider Facelift And Neck Lift Surgery?

Facelift Surgery

Patients typically request facelift surgery to address:

  • Lateral eyebrow droop that results in a skin fold masking the corner of the eye

Facelift surgery results in significant rejuvenation in each of these areas by removal of excess skin and repositioning of the muscle-connective tissue complex under the skin and fat.

This increases definition in the corners of the eyes, upsweep of the lateral eyebrows, defines and increases prominence of the cheeks, softens the nasolabial creases and marionette lines and increases definition of the jawline. It is often undertaken in combination with neck lift surgery to optimise aesthetic outcome.

Neck lift Surgery

Patients typically request neck lift surgery to address:

  • Excess and lax neck skin
  • Fatty tissue under the chin
  • Loss of definition of the cervico-mental angle between the neck and chin
  • Platysmal bands (vertical lines under the neck skin)
  • Prominent submandibular salivary glands under the jaw

Neck lift surgery rejuvenates each of these areas by removal of excess lax skin, removal of the fatty tissue under the chin, creation of an acute cervico-mental angle and management of platysmal bands and submandibular salivary glands.

Facelift & Necklift Surgery Consultation

Before undertaking any procedure with NorthEast Plastic Surgery, you must first book a private consultation with one of our experienced surgeons.

During your consultation, your surgeon will assess your expectations for your surgery and identify potential limitations to achieving your expectations, such as skin quality, facial skeletal structure, and overall facial proportions. Your surgeon will then take you through potential surgical management options for each of these areas, which will include:

  1. Management of the upper face
    Rejuvenation of the upper face include addressing forehead creases, frown lines and lateral canthal lines with injectable products or your own fat using lipofill techniques and can be considered in isolation or in combination with surgical rejuvenation of the temporal and lateral eyebrow complex.
  1. Management of the mid-face
    Rejuvenation of the mid-facial area include removal of excess skin, repositioning of underlying soft tissue structures. The use of skeletal augmentation / lipofill techniques can also have a complementary role.
  1. Management of the lower face and neck
    Rejuvenation of the lower facial/neck area addresses jowls, marionette lines, excess neck skin, platysmal (vertical) bands and fatty tissue under the chin.

Your surgeon will advise you on the impact each of these elements will have on your desired surgical outcome. Your surgeon will inform you of all treatment options, the degree of surgery required to achieve your desired results and will provide you with a range of before and after images.

Pre-operative Management

Your surgeon may request you undertake further medical examination before approving you for surgery, such as a blood tests. Your surgeon will also advise you on the medications, supplements and activities that will need to be ceased before undergoing surgery.

For example, is recommended that any blood thinning agents be ceased or minimised in the lead up to surgery. Other medications, such as the oral contraceptive pill and aspirin, and supplements such as garlic and fish oil, may also need to be stopped ahead of surgery.

It is vital that smoking ceases six weeks before and after surgery, along with any products containing nicotine.

Risks

With NorthEast Plastic Surgery, you can be confident you’re in the hands of experienced and knowledgeable surgeons who take pride in the care and safety of their patients. As part of this commitment to you, it’s important we inform you of the risks associated with any surgical procedure.

Though uncommon, complications that can arise include scarring, infection, wound complications and excessive or abnormal bleeding. Any corrective surgery to address such complications would involve its own set of risks.

Risks specific to facelift, necklift and lipofill surgery include seven to ten days of significant bruising and distinct swelling known as oedema. A significant but usually temporary risk that improves over six to nine months is sensory nerve damage affecting sensation of the ears and cheeks. A serious but rare risk is motor nerve damage that impairs movement of the eyebrows, eyelids and lips.

Additional risks include loss of grafted fat, incisional alopecia (hair loss in the line of incisions within the hair) and asymmetry which may require revisional surgery as appropriate.

Your Surgery

Your procedure will be performed by one of our experienced surgeons under general anaesthetic administered by specialist anaesthetists in an accredited hospital. Incisions are typically made in front of and behind the ears and within an area known as the temporal and posterior auricular hairline (the scalp and behind the ears), as well as sometimes under the chin.

Wounds are repaired with both internal dissolving stitches and external stitches that require removal. At the completion of surgery, tapes and dressings are applied which are suited to showering. External stitches of the facial and ear skin are removed one week after surgery and those in the hairline are removed two weeks after surgery.

Recovery period

It is important that access to medical attention is available by telephone once you have been discharged. Issues about which your surgeon needs to be made aware urgently include pain uncontrolled by medication, bleeding to the extent that the dressings soak through with blood, temperatures exceeding 38 degrees Celsius, redness of the skin or any concerns related to the surgery.

Following observation in hospital, patients can usually go home the same day. At the time of discharge, appropriate pain medication and antibiotics will be dispensed. It is important that these be taken to minimise discomfort and potential wound complications.

Patients are encouraged to keep their head in an elevated position with pillows at night to minimise bruising and swelling for one week. It is also important that access to medical attention be easily available via phone. Issues your surgeon should be made aware of immediately include fever, excessive bleeding, uncontrollable pain, skin redness and any issues related to vision or the procedure.

Facial compression garments are typically worn for two weeks after surgery followed by massage and taping of scars. You will be advised of such treatments during your post-operative visits. Wound healing generally takes from one to two weeks, during which movement should be undertaken with care. Swelling will continue to subside for six to eight weeks and may take several months to subside fully.

Patients can return to driving once they have ceased pain medication, which is usually five days after surgery, and can return to work after approximately two weeks. Strenuous exercise can normally be resumed at around four to six weeks.
